Fees and Payment

There is generally a one-time fee that covers the entirety of the evaluation. In some cases, an hourly rate will be charged if a full evaluation is unnecessary (i.e., concussion management), or for follow-up or therapy. A 10% deposit for the service is required at the time of booking, with the remainder of the payment due at the time of the appointment. This fee covers record review and work done in preparation for the appointment.

Midwest Minds Assessment and Therapy PLLC is an out-of-network provider, which means payment is the responsibility of the patient. HSA or FSA cards may be used, along with credit/debit cards, check, cash, and Zelle. Please check the rules for your HSA/FSA account to ensure that the service is covered.

Insurance companies will sometimes reimburse you a portion or all of the amount paid for out-of-network services, but this is never guaranteed. To determine whether services will be covered in advance, they will need to know the billing codes that will be used on the date of service. These usually include:

They will also want to know the reason for testing or diagnosis (ICD-10) codes, which can be provided by us or found online at CMS.gov. The documentation needed to submit to your insurance company (a superbill) will be provided upon request after the appointment is complete.
