Fees and Payment

Midwest Minds is an out-of-network provider, which means payment is the responsibility of the patient.  At our practice, we are committed to providing personalized, comprehensive care tailored to each person’s needs. To ensure we can devote as much time as necessary to your evaluation and care, we operate as an out-of-network provider. This allows us to focus on quality and flexibility without the constraints imposed by insurance networks. A superbill is always provided after the completion of the service, which you may submit to your insurance company for possible reimbursement. 

Service Fees

• Comprehensive Neuropsychological Evaluation: $3500                                      This is the charge for most evaluations, including pediatric cases, and adult Autism, multiple diagnostic rule outs, and evaluations for test accommodations. This fee covers all time spent in clinical interviews, testing, scoring, report writing, and a feedback session. Additional feedback sessions are available at a cost of $275. 

• Adult ADHD or Concussion Management Evaluation: $2,500
  Designed for cases where ADHD or concussion is the sole concern. Includes interviews, targeted testing, scoring, report writing, and a feedback session.

• Brief Cognitive Screening/Baseline Assessment: $1,500
  Ideal for aging adults or those seeking a cognitive baseline for sports or other reasons. Includes a brief interview, focused testing, and a feedback session to discuss strengths and areas of difficulty. 

• FAA Evaluations, Strengths-Based Assessments, Independent Educational Evaluations (IEEs), and Forensic Evaluations:
  These specialized assessments are billed at different rates. Please contact our office for detailed pricing information.

• Individual Therapy: $225 for a 50 minute session

• Intake Interview and Treatment Planning for Prospective Therapy Clients: $350

              A complimentary 15 minute meeting is available to determine goodness of fit 

              with your psychologist. 

• Executive Function Training with a Neuropsychologist: $275 for a 50 minute session

• Tutoring Services: $70 per hour

Insurance companies will sometimes reimburse you a portion or all of the amount paid for out-of-network services, but this is never guaranteed. To determine whether services will be covered in advance, they will need to know the billing codes that will be used on the date of service. These usually include:

They will also want to know the reason for testing or diagnosis (ICD-10) codes, which can be provided by us or found online at CMS.gov. The documentation needed to submit to your insurance company (a superbill) will be provided upon request after the appointment is complete.
